What installing outside the App Store actually involves

This is the one to be careful about. A configuration profile is an iOS management file, and depending on what it contains it can install a root certificate, route your traffic through a proxy, add or restrict apps, and change device settings. A profile with a root certificate can decrypt traffic that would otherwise be private to you.

Nothing about a profile is inherently malicious. Companies use them to manage staff phones legitimately. But a profile from a casino download page is asking for management rights over your phone in exchange for a game, and that is a genuinely bad trade regardless of who is offering it.
